We are only accepting 20 Applications! You must be referred by a well-known Agency or be a previous Client in good standing to participate. 

What you need to apply:

Proof of Income (Tax Returns are not recommended) of 30 days 

Proof of Address (NO personal letters accepted)

Picture ID for ALL Adults in the household (Valid Picture ID ONLY)

Medical ID cards or birth certificates for all children in the home.

If the children are not your natural or adopted child please bring proof of that. 

Please bring copies of the above documents:
-Id's on one page
-Medical ID cards on one page
-Proof of Income on one-two pages
-Proof of address on one page

You should have a total of 4 copies!!!!

We will not be making copies. 
Copies must be made as listed above.

If you are missing any of the above documents YOU WILL NOT BE ALLOWED TO APPLY. 

Client must be able to fill out the Application on November 16, 2019 or December 7, 2019. These are the only 2 dates available!
Appointments will take place at the 
Best Western Inn (in their meeting room)
 located on the corner of 27th and Highway 395.
